St Teresa’s School

I, <name of legal guardian>;

1. hereby give permission for my daughter, <name of learner>, to participate in the normal sporting, cultural and religious activities of the school. I realise that no claim can be made against the school for injuries, or articles of clothing and equipment that may be lost or damaged.
2. I will support the policy of St Teresa’s School with regard to compulsory attendances at school and extra-mural activities, and will not ask for permission to extend vacations nor make doctor, dentist or driving license appointments for my daughter during normal school hours.
3. I understand and accept the fee structure, my accountability for paying the fees and the methods of payment, I undertake to give one full term’s notice, or pay a full term’s fees in lieu of notice, should my daughter leave the school.
4. I accept that if this application is successful it will be in terms of the general conditions governing admissions and the school’s code of conduct (which is available from the school office, and elsewhere on this web site).

Our banking details.

School fees are compulsory and payable in advance.

Payment of fees:

• Fees are payable in the first week of the beginning of each term.
• Late payments will attract interest at the end of each month for the amount outstanding for the term.
• Monthly payments are accepted only by prior arrangement with the bursar.
• For secureity reasons NO CASH will be accepted at the school. Please deposit money directly into our bank account. These details will be ade available on request.

WITHDRAWAL NOTICE

One FULL term’s notice in writing is required should your daughter leave the school. A term’s fees in lieu of notice will be charged if this notice is not received within the first week of term prior to her leaving the school.
